A longer explanation: You don’t refer to your girlfriend or your friend or your fucking mom as ‘the brunette,’ do you? You don’t really refer to anyone you know in real life by an epithet. You refer to them by their name. 

Like, if you’re writing from the perspective of a random sailor in the 1800s, it makes perfect sense for them to refer to their captain as the captain as much as they would their actual name. Likewise, it makes sense for anybody in a monarchy who isn’t in the ruler’s immediate family to think of them as the queen or the emperor or whatever. Using a title epithet rather than a name (and vice versa) communicates something about the POV character’s view of their relationship!
