Toji thought that he managed to flee his clan and that they would leave him alone but the wrong person died and the new Zenin head wanted him back and they would do everything to him to make him come back including killing his child.

Toji tried to disappear but he wasn't fast enough and soon they were before his door ready to blow him up if he didn't come with them together with his son. He saw no way out but to agree for now and try to flee again later but he had a life changing decision to make before that:

His clan knew that he was a father but they didn't know that had two children. One of the twins he could leave behind and probably save his life and definitely save his childhood. But could he do it? And which one of them? They were so small and inseparable.

In the end he made his decision and left Megumi behind, taking Sukuna with him to the Zenin with the goal of fleeing from them again when the right time came while protecting his son from the bad things they wanted to do to him.

But when that time finally came more than 10 years had past and one of his children did had to go through and abusive childhood while he had no idea what had happened to the other one.

Then [plot], and then Megumi and Sukuna are happily making out on the kitchen counter and Toji is only "well at least they're together again."
