Episiotomies Don’t Help Moms Or Babies In Most Cases, But Remain Common
Episiotomy, a once-routine surgical incision made in a woman’s vaginal opening during childbirth to speed the baby’s passage, has been officially discouraged for at least a decade by the leading association of obstetrician-gynecologists in the United States.
Nonetheless, despite evidence that the procedure is only rarely necessary, and in some cases leads to serious pain and injuries to the mother, it is still being performed at much higher than recommended rates by certain doctors and in certain hospitals.
In one recent case, Kimberly Turbin, a 29-year-old dental assistant who lives in Stockton, Calif., is suing her former obstetrician for assault and battery after he performed an episiotomy on her in 2013. A video of the birth, with Turbin begging the doctor not to cut her, has been viewed more than 420,000 times.
Dr. Emiliano Chavira, a maternal and fetal medicine specialist at Dignity Health’s California Hospital Medical Center in Los Angeles, says he suspects three main reasons why some providers continue to perform routine episiotomies: They’ve always done them; they lack awareness of best practices; or they want to speed up deliveries.
“Certain segments of the obstetric community are very slow to modernize the practice,” Chavira says. “They’re very slow to abandon procedures that are not a benefit and, in fact, may be harmful. And it’s really disappointing.”

Episiotomies are rarely necessary.
I’m somewhere in Latin America. Many of the older obstetricians still do it routinely at my hospital. The residents and interns see it happening and disagree, but don’t say anything to the obstetrician doing it. Every time it happens I cringe with anger and disbelief, and feel that the mom is undergoing unnecessary trauma. Most of the time the obstetrician doesn’t even tell the mom that they are doing it; moms don’t make decisions about their care - medical paternalism is strong.
This needs to be put into teaching in medical schools, right from the start. Our university prides themselves on adapting the curriculum to fit best practices, but we had one lecture by a senior obgyn consultant who told us that episiotomies increase the ability to heal and reduce excessive tearing, and a separate lecture by an obgyn academic who said that cutting reduces proper healing and increases the risk of pelvic floor dysfunction.

I know ive discussed it before but. blind sex positivity is counter productive. telling girls to have sex can be equally as harmful as telling girls to be ashamed of sex. especially 20+ year old women talking about “sex positivity” to 15, 16 year old girls. even if you mean well, it can be harmful
this sounds anti-feminist so let me rephrase. the normalization of female sexuality is so so so important and a huge step in the right direction when done correctly. in a perfect world nobody would be ashamed of their sexual history or their sex lives. but that’s not how it is, and when i was fourteen or fifteen, even sixteen, i was force fed this idea that sex was empowering, and it got me into sexual situations that were consensual but still traumatizing because i was too young and too unprepared to handle them. i am ashamed of it. i live with that shame still. sex positivity is important. it is vital. but putting “be a slut, do what you want” over pastel backgrounds and encouraging girls to be a Ho or whatever isn’t doing shit. we need to teach girls how to know when they’re ready and have knowledge of how to handle situations. we need to give girls a safe way to own their sexuality. we need to teach and guide girls to make safe and educated decisions about sex. we need to teach girls that while there is nothing wrong with having sex, there are repercussions. we need to fucking protect girls

I hate it when Fine Arts students, Engineering students, Commerce students, Law students etc play down what they do when they hear that I’m a medical student - “Oh no, it’s not as cool/hard/intense as med.”
DUDE. MATE. GIRL. FRIEND.
ARE YOU HEARING YOURSELF?
You are downplaying your achievements, skills, talents, effort, blood sweat tears because you think what you do isn’t as “worthy” as medicine?
HELL NO.
You listen to me, you Arts student - we need people like you in the world to make art. Who else gives us music, beautiful photographs, design, poetry, entertainment, makes the world an aesthetically pleasing place? The world thinks it doesn’t need another artist - and this is why WE NEED THEM.
You listen to me, Law student - DO NOT pretend that you do not slave away at night with your countless readings, trying to make sense of what’s happening. DO NOT deny that you might have some dream of solving some injustice in the world - and if you don’t that’s FINE too - we don’t get into our fields always because we have some noble self-sacrificing goal to attain, sometimes the goal finds us. LET IT FIND YOU.
You listen to me, you Engineering student - just because buildings don’t give you as much grief as humans do does not reduce the amount of work you put into structural balance and beauty. DO NOT let them tell you you are unnecessary.
YOU LISTEN TO ME, FUTURE TEACHERS. I love you. I love that you will one day educate society’s future. I love that years ago a great teacher believed in me, and I still grief over that teacher who didn’t - so GO AND MAKE A POSITIVE DIFFERENCE.
DON’T be “modest” about your work because I’m a medical student. I am not mocking you for telling you that your work is cool - IT IS COOL. I could never do such a thing. So be PROUD of what you do. DO what you do. We can’t all be doctors, lawyers, teachers, artists, missionaries etc. YOU DO YOU, and stop telling me what you do is “not as cool as medicine” because YOU, and by extension, what YOU do, might be more, but certainly no less, than medicine.
When I was about to start high school, my mom told me that senior boys were grown men, and if any were interested in me then I should ask myself why they, as adults, were interested in a kid right out of middle school. And why they couldn’t get someone their own age. Be safe, kiddos. A year counts a lot more when you’re younger, and the kind of senior who wants to date a freshman is well aware of this.
