Johnny Ghost is transfem to me. Let me explain âď¸. (I will be using he/him pronouns to refer to Ghost, I'll get to why)
First of all, his name as a child was Gregory. I assume that's the name he was born with. He did change his name later on, of course. Personally, I can see the decision to start going by Johnny as an effort to fit in? He knows that the name Gregory feels wrong, and everybody likes Toast, so why not use his name?
Another thing is that he has mentioned how much he doesn't like the idea of himself wearing a skirt. In my heart I see this as an attempt to not acknowledge the feelings that portraying feminity gives him.
Ghost is this kind of perpetually miserable figure with some sort of self image issue going on, and I think his misogynistic behaviors play a factor in that. He can't even help but make a snarky comment on the fact that his male friend has a pink car; there has to be a lot of unpacking to be done before he can ever feel like it's acceptable for himself to identify as a woman.
I think about this headcanon on a multiversal scale, like, all the different possible timelines of Ghost that exist. In universes where he does realize he might be a woman, the majority of those timelines that feeling will be buried deep and ignored. I think that in a vast majority of universes Ghost doesn't even realize that's what is happening. Because of this, Ghost is almost always going to be using he/him pronouns and identify as a male, even if he finds it uncomfortable.
I do think there are some timelines where Ghost does feel able to reflect and accept her gender, and in those timelines she is a generally happier person.
