I'm a little confused, who did BH / Victor betray Sarah to first? And how so Dementia? And how was the relationship between Sarah and her husband, after she discovered it?

Victor didn't try to betray Sarah, Black Hat made him do it by manipulation and forced possession. Victor got blacked-out when The Hat started to take over his body and kill his wife and son.

After decades later, he only hired Demencia to be his bodyguard and assassin, and yes, he probably replaced Sarah with Dem.

Victor is different from BH. When Victor took the hat and got possessed and consumed by its evilness, Black Hat took over and used his body and mind.
