Thinking about an au where kokuzan has a kinda similar story to akaza and koyuki where they are born in the same era and muzans family being aristocrats wanting to hire a guard/caregiver for muzan and reached out to the Tsugikuni family since they are a aristocratic/samurai family and them sending Michikatsu (kokushibo).
Michikatsu being annoyed at first and thinking this is a waste of his time and skills but eventually growing soft towards Muzan and genuinely wanting them to get better. Muzan being closed off and feeling bad for being a burden toward Michikatsu but also loving the company of someone other than the doctors and maids who treat them like glass.
On days when Muzan is well enough to be out of bed they would sit in the garden and watch Michikatsu train or then going on walks around the estate together. Then when Muzan is bedridden Michikatsu sitting with them, maybe reading, talking, sharping his sword or just in comfortable silence.

In my brain this would be in the Heian Era (muzans era) but that might just be my love for the heian era taking over, this could also work in the Sengoku Era (kokushibos era)
Aside from the fact that I have several wips with more or less this exact theme (Michikatsu taking care of a furious yet impotent Muzan is the goat), I think you might enjoy this post. It's an ongoing rp post, but the writing is just awesome, I'm following and looking from the side like >:3

I do think little Michi would take a good care of kid Muz and fusses over him. They're both stubborn (â *â ´â Ďâ ď˝â *â )
oh their personalities would absolutely clash, especially because michi used to have a little bit of superiority complex towards people he perceived as beneath him, even if he was always very kind and ready to help (as seen in his original relationship with yori). If you combine that with muzan "how dare you look down on me" kibutsuji, they are going to butt heads a lot, at least at first, but I do think they will become absolutely inseparable once they start admiring each other's best traits
