Well, that would require you to rewrite one of core elements of Eternal Sugar. Because for a fact in her story Cookies do get hurt multiple times, and she even orchestrates that

Eternal Sugar in her quest to keep Hollyberry in her Garden was willing to hurt Wildberry, who is pretty much her adopted son. Yes, she did heal him later. She would always heal every Cookie, she's not the type of character that would kill someone. But she's not afraid to to hurt them to make them depend on her

And it's not something new for her either, she's been doing this for ages. Even to her very own followers/Cookies from her region

Like, at the core she is a person who is full of love. Her love for Cookie kind is endless. And that's why it hurts her so much that the natural joy is so short lived, and so she aims to give them the eternal version. Regardless of how stagnant it can be

Eternal Sugar's love is smothering, it's toxic, and can be even obsessive (as seen towards Hollyberry). Additionally, when push come to shove, she can lash out

She is willing to hurt somebody personally just to make them stay inside. I genuinely don't think it is her goal to ever kill somebody. But she is very aware that she does cause harm to Cookies, and Hollyberry even calls her out on it

This is your rewrite anon, so you can do what you want, but if you plan on keeping Suagr's personality then I think it would depend on who and how they get hurt to make her fully panic. Holly getting hurt would sadden her, but ultimately it would be good for her plans to keep her in the Garden. I think the best bet is if something were to happen to Pavlova, as she seemed somewhat concerned with him wanting to leave, when in the past he was very complicit with her ideas and helped her a lot. But again, her and Pavlova's relationship is also quite complicated but also unexplored in game (as far as I'm aware, still haven't watched much from second saga)
