Personally, i actually dont have an issue with people who are non dysphoric (cis) getting things like HRT or surgery. Some transmeds do some dont, most people i know dont think it's an issue IN GENERAL, IN A VACUUM, as it could be considered body modification and a part of bodily autonomy.
The problem is that, at least in the American healthcare system because that's what i have experince with, there is no determination between people who need these medications and procedures for their transition and mental health, and people who just think its cool, like a tattoo or piercing or other body mods like split tongues or implants.
A person with dysphoria is not prioritised despite the fact that it is a medical nessasity for us, and this results in the abhorantly long wait lists for one, but also an idea both socially and through many insuarnce providers that transition is cosmetic. People die on surgery wait lists.
Medically, this means many people are unable to afford SRS, top surgery, FFS, or HRT because its deemed voluntary and cosmetic by their insuarnce provider and that assertion is enabled by the plethora of "non dysphoric trans people" who see transition as just that, optional.
Socially, this has (partially ofc, not the only reason) lead to the idea that it's due to insecurity or social stigma surrounding their AGAB and should be therapy-ed out instead of receiving gender affirming care, and that our healthcare system that already tries to avoid covering as much as it can get away with, does not need to help us, because a large ammount of non dysphoric (cis) people are misrepresenting our needs.
1. A distinction through insurance AND discussion between a person dx with dysphoria and person without. People with dysphoria will have these costs covered by insurance, be either prioritised in wait lists or have clinics exclusive to dysphoric people (imo a mix of both sounds best but that's a long way down the line) and cis people (non dysphorics) who want the procedure can go about it like any other cosmetic surgery.
2. A distinction in language. The word transgender has essentially become synonymous with anyone GNC and this creates both pushback against gender affirming care, and confusions when an AFAB person going by she/they and 100% comfortable with her body is talked about as though she has any needs even ADJACENCT to an ftm/mtf/nb person.
This dosent mean she shouldn't be able to do what she wants with her body, but her medical needs should not be lumped in with ours and then end up warping them because trans (dysphoric) people become diluted by cis people claiming to be us despite monumentally differing needs, experinces, and mental health concerns.
3. Ditch the informed consent model. When I went on HRT I required a note after multiple sessions with a gender therapist. Visits to a gender therapist SHOULD be covered for everyone but the note should not be given to non dysphoric people. If they recive a diagnosis and note from the therapist, insurance and prioritised care. If they don't? It can be persued like any other cosmetic surgery if that's what they want.
None of this is fool proof and a big problem does stem from the American healthcare system being fucked up to hell and back, but even under a universal healthcare model i still think these things should apply. Dysphoric (trans) people require a dx and then subsequently recive priority in the wait lists, and to have their needs payed for by insurance, non dysphoric people do not, but can persue them on their own accord if they wish.
