A 23-year-old male experienced a fatal medical event following an episode of asphyxiation during an intimate encounter. While being choked, he slipped in and out of consciousness before suffering a seizure, loss of bladder control, and eventual cardiac arrest. Upon seizure onset, the patient exhibited body arching, involuntary urination, and an engorged genitalia, which persisted momentarily before he lost all vital signs. Emergency resuscitation was immediately initiated, but despite aggressive CPR efforts, the patient remained unresponsive, pulseless, and not breathing. His abdomen appeared sunken, suggesting muscle relaxation, while his neck showed signs of cyanosis, indicative of prolonged oxygen deprivation. During resuscitation, bruising developed on his chest due to forceful compressions, and re-engorgement of the genitalia was noted following CPR, likely due to mechanical blood flow redistribution and autonomic nervous system responses. However, his heart failed to resume effective circulation, and he was declared dead at 8:55 AM. The combination of prolonged oxygen deprivation, seizure-induced complications, and cardiac failure ultimately resulted in an irreversible fatal outcome.
