Official Voluntary Bus Inspection Form
Completion of this form is voluntary and optional, however refusal to complete may lead to increased scrutiny from the Federal Bus Management Commission (hereafter referred to as Inspectors or the Commission).
This form must be filled out in full or otherwise completed to the best knowledge of the responsible party (hereafter referred to as Driver/s).
By submitting this form Drivers agree to allow physical inspection of their vehicles (hereafter referred to as Bus/ses) at any time, with or without prior notice.
By signing this form Drivers legally state that all information entered by them into this form is both factual and complete to the best of their knowledge.
Alterations to this form not related to completion of the form (including but not limited to: tearing, burning, destructive folding, defacing, cutting, paranormal rearrangement of text, magically altering color or material, shooting or licking) are strictly forbidden and may result in punitive action or a mandatory resubmission of an intact form.

Attention all bus owners and/or operators! We at your local branch of FBMC are happy to announce that after quite a bit of setup (and even more donuts) we are finally able to begin doing inspections! Now, as a truly bizarre number of the busses in this area seem to be... unusual and scary, we have elected to forgo in person inspection, at least initially. Instead, we have made a form we’d very much like you to fill out! Filling it out (in full please!) and turning it back in to us (using either the ask function, or the submissions, if Freddie remembered to turn those on) will go a long way to helping start this long, but necessary and hopefully fun task!
