In Super Mario Galaxy and Super Mario Galaxy 2, Luigi's eyes do not respond well to his death.

If Luigi dies from being hit by an enemy attack, the stacking of his eye and eyelid textures will become glitched, causing one of his eyes to have a white pupil, while the other one will go blank and lose half an eyelid.

Due to Luigi looking down during this and the in-game camera showing him from above, this is relatively difficult to see during regular gameplay, which may explain why it was not caught during development. This was also not fixed in any subsequent release of these games.

Top: Paper Mario: The Origami King contains an unused setting for text to be displayed at a 90° angle, demonstrated here in-engine.

Bottom: the game often uses text gimmicks for characters who are in nonstandard states. As such, it is possible that this was intended to be a text gimmick for characters who for some reason or another, are arranged perpendicularly to Mario while speaking. This is a mock-up of what such a usage of this text effect could have looked like.
