You know, I think that something that could fit with that is

Like, Sam is having problems with the demon blood

And Cas offers a solution for it, that being Sam drinking his blood, even though Cas is a little weakened right now, to control Sam's addiction

So, it's their thing, Sam drinks Cas' blood at least once a day

Cas quickly shows "worrisome for Sam's comfort" and starts offering them to be in specific positions

Like, Cas sitting on Sam's lap, they laying on the same bed

Then he says that Sam doesn't need to wait the blood to drip and lick it, he could just suck it

Sam accepts it, is so refreshing to see that Cas isn't disgusted by him. Being close to Cas also makes him... Think about stuff

Time passes and, Cas starts getting flustered while they do it. He starts touching Sam, like placing a hand against his chest, caressing his arm. Sam responds by keeping Cas closer, caressing his face, holding his waist

One day, with Cas sitting on Sam's lap, and Sam feels the fabric on his thigh getting wet

He stops

Cas feels embarrassed, trying to keep a straight face, asking for Sam to continue his activity

As Cas gets weaker and weaker, he is concentrating his grace on the blood repositioning, and now some body fluids like sweat and, in the case, lubrification, are bound to happen

He says he's sorry, he can't avoid his physical response

That they can change the method into giving Sam the blood indirectly, like in a glass or bowl, but

Sam kisses him. And Cas kisses him back

Now, their usual activity is accompanied by making out sessions, and some more adventurous touches

But there's one detail: actually, drinking Cas' blood isn't the only way of controlling Sam's addiction

He could have suggested to realise a weekly spell to keep Sam's desires down

It would have almost the same effect

But, Cas doesn't think it is essential information

They're doing very well by now
