I'm planning to include all the explanation in the first part but BASICALLY, in ancient times, palace rules dictated that any woman within its walls belonged to His Majesty (this includes court ladies and female physicians alike) but it doesn't necessarily mean in like a romantic way or anything (unless the King likes any one of them like that, then they'll be promoted to a concubine). So, women working in the palace are usually not allowed to be married unless they retire and leave the palace when they grow old.
At least this is what I understand from my K-drama knowledge and through a bit of research HAHA hope this made sense!🫶🏻
