So, something I noticed and that's been racking my brain for a moment because I genuinely wondered "did I hallucinate it" is the fact that Shichibi's appearance before the fight and after the fight in FWT is different. Before the fight, he looks like a red Kyubi but after the fight, his eyes stops glowing and he looks less threatning.

So I had to actually rewatch the boss fight and pay attention and I noticed that, during the fight, the transition happens

Each time Shiichibi reverts to Fox mask, it loses the red color, and when it's completely washed out, it's the exact moment where Kotoyuki gives up on the fight and accepts to let Hinako go

And then I went to read Shichibi's description

I think the festers might be a visual representation of the "curse" but in that case, it means that Kotoyuki/Shichibi is losing these as the fight progresses, which might translate in him taking awareness of Hinako's feelings ? Maybe ?
