i would say cause all of s4 but tfp specifically was sherlock working through his fears regarding emotional attachment and its potential consequences (the constant guilt of putting his friends in danger because they can be and have been used as leverage against him) and by the end he finally learns to reconcile that fear and fully accept that āit is what it isā, itās worth fulfilling relationships even if theyāre risky. john was symbolic of romance and greg was symbolic of friendship.
so naturally if there was ever a time for sherlock to remember gregās name it would be at that moment, after this emotional revelation. we might not ever hear it again from him lol, itās safe for him to say that to mind palace lestrade, but he does love to be annoying so he prob will still pretend to forget his name irl lol
also, most importantly, if it turns out it was just johnās mind making sherlock say "gregā, then that takes away all the significance of that line. and that line was written and delivered to carry weight

I guess I have a hard time with this because I have borderline personality disorder and I tend to split on people (especially people i admire) if I feel they've done something unforgivableā¢. I know rationally it's their business and it was probably an amalgamation of things. It's just a little bpd trait of mine that tells me he's evil if he did something I don't personally agree with.
