I think he manages fine! I just don’t like the headcanon that Sasuke never accepted the prosthetic “to remind himself of his sins”. It doesn’t make sense, what with the whole journey of redemption and all. In my mind he accepted it but his body rejected it in the end, so he just ended up not having one.

For himself, he’s not a super big fan just because he’s impatient and doesn’t like to be teased, though he’d be lying if he said that he didn’t have very powerful orgasms when it was done to him anyway. He definitely likes to do it to Sakura though, when he’s really in the mood to tease, and he will draw that out for a long, long time until she is begging him and/or gets pissed off. He loves to have her so needy and riled up, especially because he knows how insanely good her orgasm turn out to be when he finally lets her have it. Sometimes Sakura can manage to grab that orgasm before he’s ready to give it to her tho---if she gets frustrated enough, she uses her strength to flip them over and ride him to her end without him being able to do anything about it. He’s never annoyed though, always just endlessly amused by how needy she ended up being.

Pre-shippuden, Sasuke obviously never thought about sex. He knew what it was of course, all ninjas having had sex education rather early on, but he didn’t think about it in regards to himself because revenge was obviously too important, and he was too young anyway. When puberty hit him in shippuden though, I think it’s very possible he may have had wet dreams, with flashes of pink hair and green eyes always somehow on the forefront, but he probably didn’t think much about that anyway, if he’d remembered it at all. Revenge was still be his main focus, if not his only, so I doubt he had much manifestation of his needs apart from wet dreams.

Post-war, though, that’s where things changed. It was still not something he thought about when their relationship was at such a tentative, innocent stage, but I think Sasuke probably noticed things a little more. Not that he thought about sex, but I’m sure his desire started to take root somewhere there. Noting the curves of her figure, the smoothness of her skin; trying to imagine what it’d feel like to touch her or to have her touching him. But of course, those thoughts would quickly be shaken out of his mind because it would be so improper to think about her like that. 

But then of course their travels would take place, and somewhere down the line, he’d start noticing things about her that really would send his blood simmering---like when she’d stretch and he’d catch sight of her bare, flat tummy, or when she’d tie up her hair when she’s all sweaty and he’d zone in on the damp, flushed skin of her neck. He’d think about what it would be like kiss her there on her stomach, to bite down on her shoulders, her neck, while he’d run his hands all over her and---oh, the tension that would build when he’d think about those things more and more, always reprimanding himself for it. Until, of course, the desire became blatantly mutual, and it started being a little more flustering rather than shameful, until their first time finally happened and they finally had a release. I imagine though, in those times the tension would build... man, every day would have been an absolute mess of hot thoughts.

As for how long he can go without sex, I think he’s really the needier of the two. As explained here sex is just very emotional for him and the best way he knows how to show his affection, so he yearns for their lovemaking a little more than she does just for that reason.
