Perhaps in the absence of the ability to take care of their own kits they start exploring their identity, this probably was started by them leaving the clan for just a bit, but after they had kits and weren't able to take care of them themself (thus making it feel like a part of themself was missing) they sought to figure themself out

Perhaps it was sitting on the riverbank with Mothwing that started it all. Mothwing discussing her own identity, her own beliefs and feelings. Talking about how she didn't need a sign from StarClan to know she was a girl. And Leafpool thinks about who she is. She's a medicine cat, isn't she? But what else is she outside of that. What else are they outside of that?

Perhaps it was the time away from her sister. Squirrelpaw and Leafpaw, two sisters, two daughters. But is that really what they are? Maybe Leafpaw is more than just her sister's other half, Firestar's other daughter. Maybe the issue is the title, maybe the issue is themself.
If Leafpaw knew... I think Cinderpelt would tell them about her own thoughts on it. How you don't need the stars to tell you who you are. What you are. it's important to listen, but it's important to carve out your own path as well.
