EHCO Day 4

February 27th, 2018

After eating some yummy pancakes in honor of National Pancake Day (s/o to everyone that got up earlier to make them!!), we were on our way to the first stop of the day: St. Jude Children’s Research Hospital.

Arriving at St. Jude around 8:45 AM, we were greeted by Thonda and Dennis of the Leadership and Development Department. They prepared blue & maize name-cards and name-tags for us, and walked us into a conference room as the Michigan fight song played in the background. It was very cute :)!

Thonda spoke a bit about emotional intelligence before Dr. Downing, CEO of St. Jude, entered the room and began an inspiring talk about the hospital, his career path, life and leadership. He emphasized collaboration, teamwork, and fairness in terms of what makes a good leader. We were very lucky and grateful to have him with us for around 45 minutes instead of the planned 10!

(Thonda summed up his talk really well: “...not only does he have the ideas, but he can communicate them and inspire.”)

Following Dr. Downing’s departure, there was a presentation on the Graduate School of Biomedical Sciences given by Brian Walton, one of the associate deans.

After this, we were introduced to two panelists: Colette Hendricks, VP of Clinical Operations and Dr. Thomas Merchant, Radiation Oncology Chair. They explained their journey to St. Jude from undergrad, and gave us some useful life advice.

Thonda continued her presentation on emotional intelligence once the panel ended, and then it was lunch time!

Some large amount of sandwiches, pickles, chips and cookies later, we were led on a tour of the hospital by Dr. Myron. We got to see children’s artwork along the walls, toured various labs, and we were able to try out a virtual reality brain/blood vessel set-up.

Around 2pm, we took some final group photos outside, and Thonda and Dennis sent our group off with St. Jude bags, which included a coloring book Thonda designed. (Again, so cute.)

Next, we visited Grizzlies Prep and split up into two groups to be led on a tour by two eighth graders, Philip & Ben. They explained how their school worked—from uniforms to reading requirements—and showed us various classrooms, the cafeteria, and the library. At the end of their school day, we were once again split up into groups to have Q&As with three 8th grade classes. The students were all very friendly and had a lot of interesting questions—I wish we could’ve stayed longer!!

Later in the day, we had dinner at Crosstown Concourse and listened to presentations on finance (financial aid/credit cards). It was nice to sit down with other members of the community and hear their thoughts on these topics.

Our long day continues with a trip to Bass Pro, which is extra special here because it’s located in a giant pyramid. We walked around, bought some souvenirs, and played an arcade shooting game. It was really fun—would recommend!!

Finally, we made our way back to the bunkhouse. (&At this point we had been out over 12 hours!!) Half of us went to Luke’s place to read to his children, and the rest stayed and chilled.

The business committee has now had several productive meetings with our mentor, Randy. He’s given us a lot of insight into the logistics of how our table will be built and shipped from Ann Arbor, Michigan to Nicaragua! Based on this advice, we’ve begun to research potential manufacturers and develop criteria and a ranking system for manufacturers. We have split the business team into three sub-committees for transportation, manufacturing, and partnerships, in effort to focus on refining the most pertinent aspects of the business plan. The manufacturing sub-committee has unfortunately reached a standstill because we are only allowed to use our grant money through U-M’s Procurement Services. The team is deciding whether to go through this system or work around it in order to manufacture our table. The transportation subcommittee is working on finding a potential “agent” in country who will have an import license and be able to collect our tables once they reach Nicaragua. They have also been working on mapping out potential distribution chains which will help us visualize where our tables are going and fill in any gaps we may have missed. The partnership committee has been reaching out to our current partners to find out which ones would be interested in a table for our pilot launch. Overall, a lot of exciting progress has been made!

The Table team has been cranking away with final designs for the stirrups and a locking mechanism for the legs. The locking mechanism that we are most likely going to perfect and use is a joint made of two pieces of sheet metal connected by a rivet. In addition, there is another hole in which a pin will be inserted so the two pieces of sheet metal can lock in place. This should keep the legs from buckling in and from splaying out. The stirrups mechanism has been a little troublesome. As of now we have created a telescoping stirrup system so that they can be adjusted. However, there is an issue involving wiggle room due to the telescoping square stock not being a perfect fit. This is something that we will be working to correct. Another aspect we have to decide is the stopping block for the stirrup. We ran numerous rounds FEA in order to find the correct thickness and length of this stop block. We wanted to make it hollow in order to minimize weight. So far the only dimensions that worked for a load of 50 lbs and 100lbs is .12in thickness, 2in length, 1in width, 1 in height. We still have logistics to hash out, but now we have a decently clear path to follow in order to create our prototype. 

New designs and local user feedback

The past month has been a busy one for The Initiative. We began our semester with a rather serious setback when we discovered that over the summer all of our prototypes had been thrown away during a cleaning of the Design Lab in the Dude. However, our team decided to look at this as an opportunity to redesign our bassinet and make some serious changes to our initial design. Based on a meeting with REFRESCH, an organization which aim to solve challenges across the globe by combining the skills of a multidisciplinary team of professors, we decided to make our bassinet enclosed. This was something we initially had wanted to do but we weren’t sure if it would be feasible since we needed to maintain air quality for the infant. Luckily, REFRESCH had a similar design which they had performed testing on and found that there were no problems with air quality. We decided to piggyback off this design which is shown here and look forward to prototyping it in the upcoming weeks.

Another exciting development was taking our KMC carrier prototypes to a mother’s group, Ann Arbor Baby Beginnings, to get user feedback. The mothers gave us positive feedback on our choice of materials, the security of the prototype and the novelty of our two-part design. They also gave us several design change suggestions including using snaps instead of buttons and taking into account the size of postpartum mothers. We look forward to implementing some of these changes in the future.

One of our team leads, Connor, has been working on the heating pad as his senior design project, particularly the control system for our heating pad. This has been a super helpful for us as we are seeing rapid progress on this element. Moving forward we are hoping to see it integrated into the rest of our design and testing the heating pad to make sure it is meeting our user requirements.

Trying out Guatemalan food in Ypsilanti 

So far in the month of March, Team Guatemala (name change still in the works) has continued to strive towards our goal while also bonding as a team.

This month’s social, which was the second in team history, was a trip to the beautiful town of Ypsilanti. After a great recommendation from an anonymous source, we visited Encuentro Latino restaurant. This local restaurant is owned by a family from Guatemala, and they served authentic Guatemalan food. After waiting for everyone to arrive (some of us actually got there on time), we ordered drinks and food. Personally, it was my first Guatemalan meal, and I was very impressed. My dish was simple and delicious. A few of us ended the night by ordering some tres leches, a common dessert in Guatemala. Not every night can be a cultural adventure however, and once we got back to the university, work on our project continued.

Both of our sub-teams, concept generation and design specifications, continue to work hard. Design specifications sub-team is working to finalize the user requirements for our device. One way the sub-team did this, was by getting feedback from an M-HEAL advisor. We integrated this feedback and generated questions that needed to be answered to further our work on our specs. We also contacted a medical student (and good friend) in Guatemala, Andy, and asked questions specific to the hospitals there. Next steps for this sub-team is to contact more people in Guatemala about our specs and talk to more people here at the University to finalize them. Our concept generation sub-team has been working on multiple different things in the past month. One thing that they are doing is organizing their current concepts by categorizing and refining them. Also, the sub-team is continuing to generate concepts and benchmark current solutions. Lastly, they have begun to contact consultants to discuss the process of downselection.

Future: We will continue to finalize our design specifications while refining concepts and hopefully have one last social before the winter term ends!

Uncovering challenges in building and research

During my time spent so far working on the PeriOperative project since January 2017, I have become well accustomed to the many nuances and intricacies involved in this project. As with any project, things always seem much simpler from the surface, and it's not until you take a closer look at the project and try to build it that you realize these challenges, and this has proven to be especially true now. At first, the idea of implementing a simple resistive heating element and power supply seemed like a simple task. However, I have found it to be incredibly challenging to do while keeping safety, cost, and durability at high priority. One of the major challenges has been finding suppliers who are willing to work with a small project since many have minimum order sizes that are way above our needs. Another challenge has been trying to keep the system as safe as possible, which places many constraints on the type of heating element and power supply we can use. An interesting challenge and great learning experience has been researching and deciding between all the different insulation types, conductor alloys, and wire configurations (combinations of parallel and series connections). Additionally, this project has led me to do extensive research into the various safety mechanisms that can be implemented into our electrical system and into the National Fire Protection Association’s National Electric Code (NEC), which has been very useful in guiding me to engineer the safest product possible. The skills and knowledge that I have been gaining from this project are incredible and will be useful for my future career and daily life.

The Neonatal Asphyxia Project started the winter semester with benchmarking, in which we researched current technologies and devices that exist to treat neonatal asphyxia. It was quite an extensive and arduous process, but we are proud to say that we have finally finished our benchmarking document, which includes detailed analyses of fifteen different devices! We hope to take this benchmarking document to an advisor to try and find our niche in a very saturated market.

I’ve learned so much during my 1.5 semester in NAP, but one of the most important lessons I’ve taken away from my experience thus far is that the design process not a walk in the park, and it’s certainly not the idealized, seamless process I had imagined coming in. There are several obstacles along the way, and NAP has encountered its fair share of them. While we were planning to travel to our contacts at Hospital Escuela Universitario in the Honduras next summer, we recently discovered that there are travel restrictions for that area of the Honduras, which means we won’t be able to travel there. Currently, we are looking at other options, either travel to alternative locations or parts of the Honduras that are not restricted. We are also planning on reaching out to other M-HEAL teams for contacts in different countries who may be familiar with treating neonates.

We also realized that it may be difficult to develop and test our original design — a cooling device to treat neonatal asphyxia — in the U.S., as it would be classified by the FDA as a Class II or Class III device, which involves a lot more regulation than a Class I device. Therefore, we are considering slightly switching gears to find a niche that would not be under such high regulation.

While these obstacles have been, at times, discouraging, they are valuable in our personal development and our experience as engineers. I’ve realized that engineering involves a lot of time at the drawing board (and discovering problems and going back to the drawing board), and I think it’s good to have that dose of reality early on in my engineering career. Despite these setbacks, our determination and spirit has not faltered. If anything, NAP is more dedicated than ever to improve our team and focus our goals. After all, as Kelly Clarkson says, “what doesn’t kill you makes you stronger”.

Since winter break our team has been making major progress towards our goals for the semester. We have divided into subgroups that all specialize in different areas of the project. I think that this has made us more effective. Our meetings on Thursdays are now used to catch each other up on what each sub team has been doing instead of trying to accomplish things in such a short period of time. Another big development that has occurred this semester is funding. We were rewarded the BLI grant, which will help us in numerous ways. It will allow us to buy materials in order to construct a prototype and gives us more flexibility in terms of trying different things.  We have also had social events that have helped us get to know each other better. The new funding and faculty advisors we acquired from last semester has everyone excited about what we can accomplish.

 All of the sub teams have different objectives, but one of the major areas of focus recently has been IRB approval. IRB approval would allow us to collect data from the hospital in order to test and refine our algorithm and prototype. This will be a long process but getting started on it earlier will help us in the future. All in all, the rest of the team and I are very happy about our progress this semester and have high hopes for what’s to come.
