when the medi guns are just healing, they're functionally identical, both doing 24 hp/s of healing, up to a cap of 150% max health. to me, this justifies why the vax and the qf have more unique models, backpacks, and effects, because both of them have unique healing mechanics (the qf's faster healing and lower overheal cap, and the vax' slower overheal and damage resistance)

i think if it was any more obvious that the medic had kritz equipped, you could just yell "HEY THAT MEDIC CANT DEFEND HIMSELF, JUMP HIM" and then the medic is a sitting duck, or you can be more mindful about kiting his kritz target before it's even popped. the kritzkrieg's downside of not having any survivability effects for the user is rather large, and justifies the visual ambiguity. nothing is really stopping you from doing this in current-day tf2 if you do happen to notice the different nozzle on the medigun.

importantly, when kritz uber is popped, and its unique mechanic is active, there can be no confusing it for the medigun any longer. the medic shouts a different voiceline (shared with quickfix and vaccinator), and the kritzkrieg and its benefactors weapon both light up like a lightsaber, emanating crackling lightning bolts, and the resulting projectiles all have louder sounds and bright particles surrounding them. this is immediately differentiable from the metallic sheen and glowing eyes that result from a stock medic uber. you can even tell them apart before seeing them, because the medic shouts different words!
