Just finished the book and there's quite a lot of time between where the past storyline lets off and the present, and we don't see where the witch king title came from, so while it seems like Bashasa lived to see the end of the rebellion, it's possible Kai might've worn his form after his death, whenever that happened.
I am enamored by a dramatically tragic AU where he gets killed partway through and Kai has to pretend to be him though.
