I think Dwalin would be the harem AU eunuch, which translated literally means eune, "bed," + -ekhein, "to have, hold", so it just means harem guard. Since most men with harems didn't trust other men around their collection of sexy women they would castrate the guards, but I think Bilbo would let Dwalin keep his balls. It's not like there is any danger of knocking one of them up.
