I'm going to have to end treatment with a specific institute soon. I've been seeing my current therapist for about 4 years now, and the prospect of ending a relationship with her is really painful. She's helped me so much, and has been my only friend. I'm embarrassed and ashamed of my attachment to her, because I'm afraid it's unprofessional. But I'm having trouble coping with losing the only person I trust. I'm not sure what to do, and how to express my fears and gratitude to her.

I am so sorry that you are going to be ending your treatment soon and leaving your therapist. I know how hard it is to open up to people and find someone you trust so when that comes to an end it is always hard to move on.You aren't alone in the way you are feeling because I felt the same way.
You have been seeing your therapist for an awful long time, 4 years of seeing the same person and then not seeing them again is going to be painful. It has been a routine for you going to see them and obviously they helped you so much. Having the only person you've been close to after 4 years leave you isn't going to be easy and is going to hurt. She has helped you through your problems, been there for you and you trusted her. It'll obviously be difficult for you to find someone like her who would help you as much as she has.
Please don't feel embarrassed for the way you feel. It is understandable that you'll feel the way you do after all this is the only person who has been there for you and helped you. They are your only friend and losing them would cause you to feel the way you are. I am easily attached to people too so I totally know where you are coming from but I do think that feeling attached to some degree is normal.
I know you are scared of losing her and are having trouble coping but you don't have to completely lose her. I am sure if you explained your thoughts and fears she will find ways to keep in touch with you. My therapist did the same with me, we exchanged emails often and I even met up with her once or twice so she could see how I was doing. Some therapist may not be comfortable with that but I don't think her giving you an email so you can still chat now and again would be an issue.
If you are struggling to express how you feel then I would write it all in a letter. Writing it down in letter form is much easier than verbally and it will make sure that you have everything down you want to address. You can either read off the letter or give your therapist the letter to read herself. Then you can both discuss those things after she has read it.
I would also maybe get her a thank you card and maybe a small gift if you can afford it like some flowers or her favourite chocolate just to express your gratitude.
