It's a heavy topic that often goes underutilized in creepypasta fandom, I'm here to headcannon about it
Eyeless Jack
He's hungry, usually.
His body cannot subsist on anything that isn't a human product. Human meat, bones, blood, and milk (if he can get his hands on it) are the only things he can eat.
Everything else is toxic and tastes horrible.
As a surgeon, he can remove organs, such as a kidney, that his victims could live without, therefore leaving them alive, but this takes time and doesn't yeild alot of meat.
Jack isn't opposed to killing for food, but subconsciously, he feels horribly guilty about eating people. He has something of an eating disorder, wherein he often eats the bare minimum to keep going and sometimes can't eat at all.
He swings wildly from hating to embracing his inner monster. On the other side of the spectrum, he enjoys taunting and chasing. He has the demonic urge to hunt, though he's never cared for athletics.
Usually, he'll just stalk and tease his victims if he wants to 'hunt' but actual hunting is on the table for him.
Jack will kill people 'for business' as he calls it (old habits die hard), but he usually scraps the body for parts and makes it food anyways.
Dr. Smiley
Smiley considers his kills 'mercy kills' or 'euthanasia.' He doesn’t even think he's harming them.
Or so he says.
Smiley is a very old demon; he's lived in many human bodies. When he was younger, he cultivated a love for humans and a desire to care for them—something he still maintains. But over the years, he's seen alot: war, epidemic, infection, everything you can imagine befalling someone in colonial America.
He's traumatized, to be honest. He's seen alot of humans die horrific deaths while he, a doctor, stands by being useless (especially before modern medicine and sanitary conditions). He's had alot of patients die under his care. Sometimes it was inevitable, sometimes he made mistakes.
He can't take it anymore, the long nights at patients' bedsides, hoping and praying he didn't fail them, thinking maybe if he watched over them, he could fend away death itself. He's a doctor after all, it's his fault people die, he failed them.
What really broke him was his demonic hunting instincts developing. The sudden urge to rip into anything weaker than him did not couple well with his medically inclined imposter syndrome and he snapped.
He killed someone. Genuine, honest to god killed a perfectly healthy human. He may have gone on a tear for a bit, only justifying his actions with his brand new theory that most humans would be better off dead. That there's so much pain that they really should just die. The death he couldn't fend off wasn't a bad thing. It was a permanent miracle cure for all the ailments of the human condition.
The only ways to avoid Smiley, really, are to 1. Be perfectly healthy and happy or 2. Refuse treatment (he will not operate without consent, but notably, he never tells his patients what they are consenting to) or very rarely 3. Convince him you are genuinely happy and have a reason to live
Smiley's method of murder is... medical... he likes to vivisect his patients. Awake. He says the release of death is sweeter when in horrible agony immediately prior.
Everything Smiley says about his practice is a lie. He knows deep down that he's hurting them. He knows he only says that about vivisection because he likes doing it and doesn’t want to feel bad. He's just really good at gaslighting himself because he can't handle the notion that he's a monster.
Jeff The Killer
Liu Woods
Love of the game baby
He and Liu used to kill animals together. Liu just likes mutation, but Jeff is just The Killer
For him, it's all about tearing into someone and watching them die. If anything, it's about feeling powerful. Nothing more to it.
No matter how he's feeling killing usually lifts his spirits and he tends not to bypass the opportunity to kill, but it is common for him to go a while and then go crazy for a bit before getting bored and doing something else.
Sully Woods
He doesn't necessarily want to kill people but it's inevitable with what he does want, which is a gore fest. He loves gore and carnage. He finds it beautiful, sometimes he goes so far as to call his kills art
But he also recognizes that he really is just a sicko who likes mutilating people. He doesn't have some sort of grand philosophy, just that he likes gore and is a selfish bastard who is willing to hurt people for kicks.
Despite his ruthlessness he very rarely kills people. It's just a hobby, and a very taxing hobby at that. He probably only kills a few people/households a year. He tends to gravitate towards killing when he's upset or stressed since he thinks it relives stress best.
He doesn't know limits except for like, non aggressive pets and small children. Not really for a moral reason, with pets it's his only rule from when he was younger that he hasn't broken and doesn't want to break his streak. With children his paternal instincts are just to strong (won't stop him from orphaning them)
Liu is the only homicidal one in his system (other than Sully but he's technically a separate entity) and most of his other alters don't like that he's Like That, the rest are neutral but hate when he makes them deal with it (And then it's usually Sully’s problem)
Accidentally, usually.
He doesn’t really like to kill people. He's more of a tormentor. People just tend to die when you push them too far.
Sometimes he gets attached then feels bad when he kills them. Sometimes he drags them back to Smiley for help (bad idea). Sometimes he tries to keep them (doesn’t have a place to put them, also a bad idea)
He's a young demon, so his human killing is less of a malicious thing and more of a child trying to play with small animals and not registering that things die when they are killed.
He only kills with malice when he's threatened or really pissed. Sometimes he tries to just hurt people a bit and accidentally kills them anyway.
Kagekao
Save wavelength as Sully. Dosn’t care to kill people, just really likes tormenting. Not his fault his victims can't handle him.
He just gets bored. He's also just a young demon; he's kinda like a cat hunting for fun. He doesn’t make a habit of eating people (he is an obligate carnivore; he will eat dead victims if desperate, but he doesn’t kill with the intent to eat. He just gets hungry while 'pranking' sometimes), but it's nice to chase them.
He chooses to kill humans because they're smart and make the most interesting prey. He often doesn’t even kill people. Just fucks with them. He just gets a little too silly sometimes.
He never really feels bad when he kills people, though. He may get upset that his plaything broke, but to him, the best part of death is seeing others react to it. Very little is more interesting to him than grieving loved ones.
Kage is not a strong demon so he usually chooses flight over fight, but if he thinks he can take something, he will attack to kill. And he's a pretty damn good fighter.
Kagekao can have a little vicious, blood thisty murder. As a treat.

Hmmm request that a fuse of Dr. Smiley Creepypasta and Ben Drowned Creepypasta. I’m not sure if you can do that, basically mashing two characters into one. I imagine the design would be absolutely cool. Good luck!
Art trade for you: https://bsky.app/profile/joanne-nguyen.bsky.social/post/3maopegidvk26
Challenged accepted (I’d like a Jason The Toymaker x Dr. Smiley, my ship)
Rare pair!!
And a little note, for the anon:
I will start off and say thank you for the drawings! (To anyone curious, they drew NinaHeart and BlindRed)
However, this is not how I usually do art trades. If someone is interested in doing one with me, I message them (or vice versa) and we talk about what should we draw for each other. That way we can send each other references and details about our characters to help us out with drawing them.
I’ll let it slip this time. I’m not mad, don’t worry! It just surprised me. Remember to keep this in mind next time if you ever want to trade with me again.
