While I don't like the idea that he would have been 'fine', if that were not the case, he would have been much better off, if he hadn't worked his body to death, past its limits.
I'll start with Umbrella and their 'work ethic', to get to my point. They outright call for 'obedience' from their students in the training facility, wrapping this idea into a sense of 'unity' and claiming that is what will give them 'power'.
This is a pretty typical fashy sentiment used to brainwash people into accepting a role of a drone that serves the bigger picture. They are told they are part of it, but they really aren't. The 'power' that comes from their work, can't be for everyone, it can only be for the select few, at the top.
People at Arklay and the NEST, were worked to death. They had insane deadlines and practically (and at times, completely) lived at work. In Wesker's case, I don't doubt he was *raised* that way, from the start.
He was able to fulfil several very demanding roles at the same time, between being a head researcher + an assassin for Spencer, then being in the intelligence agency + the military, and then still working for Umbrella as a head of security + managing STARS.
And even when he attained his 'freedom', he couldn't really bring himself to slow down, because he was so used to always doing something and now he likely felt he had even less of an excuse to be tired or unwell, because he was a Tyrant.
This shows well in how he is willing to just tank all damage that comes his way. He'll block blades and bullets with his bare hands and just let people hit him. Even if he can regenerate, it's obvious he still experiences pain and just takes it.
Which finally brings me to the virus. His strain WAS supposed to 'dampen' over time, as it was naturally doing. Whether he 'adapted' to it or not, it WAS still an invasive, destructive force in his body, and his rare mutation fought it off enough for it to not kill him. But, that means, as an invasive disease, it was going to metabolise and eventually fade. And he couldn't let that happen, because he had become so reliant on the power it gave to him.
Between his unwillingness to grow weaker, and the wear and tear he put his body though, it's unsurprising he needed enough aspirin daily for about 5 people with heart failure (not an exaggeration, btw) + a nice dose of pain killers.
No matter his regen factor, it would also fade and weaken with age, just as our natural healing and immune system weaken over time. Even the healthiest people that are very athletic and in-shape will start to feel aches and pains with age, especially if they hurt themselves in some way when they were younger.
Our body's resources are finite and Uroboros was meant to be what would rid him of that one 'weakness' that was seemingly very rapidly catching up to him, because like I often say, he had no compassion for himself. He was raised to be a drone to a regime and even if he thought he got away from that fate, he till ultimately lived and died as one.

Scrambled tofu and turmeric stirred in melted nutrali vegan block with garlic and leftover chard and spinach salad bag remains. Sprinkled with fake bacon made from coconut flakes fried with tamari. Very bad fried bread substitute because the only gf bread in the house was stale wraps. Am now full of protein. Treated to a light lunch at @themalago today. Rather delicious vegan pesto on my Parma ham salad and they also offered vegan mozzarella instead of the cow version. They're very accommodating if you need #dfgf options. It's one of the sunniest places on north st to sit outside but today it was crazy thunder hailing and we watched the awnings tuck themselves away from the wind as we sat nice and cosy inside.
