C/P element 57, resize 50, duplicate mirror, flip, duplicate free rotate right 90, duplicate, flip
C/P element 67, resize 20, duplicate, mirror, duplicate, flip, duplicate, mirror, merge these layers together, duplicate, free rotate right 45, resize 95
C/P element 33, resize 30, duplicate, flip, duplicate, mirror, duplicate, free rotate right 90 duplicate, mirror, flip
C/P tube resize as needed
C/P element 39, resize 60
C/P element 3, resize 40
C/P element 5, resize 35
Add drop shadows
Copy merged, paste as new layer.
Use radial blur with the following settings:
Set properties to soft light
Add name and credits

Just to make it easier on you and me both, I shortened the instructions in my tuts a bit, abbreviating the most commonly used actions.
My abbreviations:
C/P - copy/paste
s/f/d - select all, float, defloat
FF- flood fill
MW-magic wand
ppr-paper
d/s-drop shadow
