Seeing this, my immediate thought was, BunAl Mori gets deaged and somehow ends up in the BSDverse. He then ends up getting chased by the Port Mafia. BSD Mori ends up forcing BunAl Mori to work for him (since BSD Mori has important things to do and it seems that he doesn't really do too much doctor work anymore now that he's the Port Mafia boss).
The other option would be to shove in Saitou instead of/or with BunAl Mori. (Actually now that I think about it having Saitou get stuck in the BSDverse may be more interesting since he seems more sheltered than BunAl Mori and also Saitou is a psychiatrist while BunAl Mori is a former military doctor)
