What are the steps to become a doctor after 12th in 2023?

Last week our applicants for medicine and related subjects went through a rigorous schedule of mini mock interviews with a number of teachers and expert volunteers. Most found the experience quite disconcerting as they were put through their paces on a range of issues. My topic for discussion was the biggest challenge they thought was facing the NHS. Covid recovery, the obesity epidemic and funding generally were all raised as valid issues for concern. If the boot had been on the other foot, I might have talked about the numbers of qualified staff we educate as a country.

Many of them talked about Brexit causing the loss of a number of qualified staff coming from European countries. Before the Iron Curtain fell and those Eastern European countries started sending us their workers, it was the Commonwealth we were relying upon to help staff the NHS. Having worked with Sixth Formers applying to study medicine for close to 20 years, I do not understand why we still are not able to train enough homegrown medical staff here.

The system is doing our potential medics a huge disservice. An applicant for medicine has to do enough academically to have predicted grades which are A and A* but these are just predictions and since AS levels went, there has been no measure of attainment in Lower Sixth which can be thought of as reliable across all schools. Applicants have to invest hours and hours across a range of different work experience and voluntary placements, getting to know the sector in which they wish to work. Rightly so – and for some it may open their eyes to what medicine is really about and they may change their minds. But for those who remain committed to medicine, are achieving great things academically, and also have the personal qualities to succeed, there are still no guarantees.

When I was a Head of Sixth Form I wrote to UCAS to complain about the cruel and unusual application system for medics. Nothing has changed since then. Applicants submit their forms by 15th October. They take the BMAT test two weeks later (this year’s computer test saw a nationwide server failure) – this is a key determinant for some universities. Others ignore it and rely on a different aptitude test, the UCAT, normally sat before applications go in. Some just rely on the application and an interview. Then the wait begins. While other subjects process their applications and many of our U6th rapidly receive five good offers, the medics wait for an interview date to come through for any university. When I wrote to UCAS all those years ago, one of my pupils had had to wait until May to hear from his final university. Many of the strongest applicants get only one or two offers. Some receive no offer at all, and have to reconsider – do they spend a gap year and apply again, or try to follow the longer, more expensive postgraduate path? There are never places in clearing, even if you achieve straight A*s. Many of these young people then move away from the dream of being a doctor, never having been given a fair crack of the whip. In some cases they may take up other healthcare roles but many will follow completely different paths and be lost to the profession forever.

We could have trained so many more of our own doctors to staff our health service during this pandemic, and beyond. Of course, training doctors is expensive and takes a long time, but it is ethically troubling that we allow other countries to do it for us. With our commitment to international development we could be sending some of our own young doctors to developing countries to learn and to help where ratios are worse – I know this goes on to a limited degree but we could do so much more if we had even a small surplus of young graduates.

Perhaps the pandemic will drive change: more medical schools are offering increased numbers of places since 2019 and I very much hope that all our medics get the places they deserve.

For any younger pupils considering medicine as an option there are some things you need to be doing:

Work at your academic studies – GCSEs will be the only hard data medical schools have on you apart from their own aptitude tests. Successful applicants need mostly 8s and 9s. In the Lower Sixth, you will need to do Biology + Chemistry + one another (the third subject varies widely these days). Do an Extended Project (EPQ) with a medical theme – research an area that you care about. Attend all the enrichment activities offered.

Take opportunities to work in a team. Think beyond sports (although that’s a great way to tick this box). Involvement in a band or the cast of a play, on a school council or volunteering with a group outside of school could be the way to go. Year 11s get a huge amount from the NCS scheme in the summer after GCSEs.

Start work experience and volunteering placements early – don’t wait until Sixth Form. The summer after Year 10 is when you want to begin thinking about this. Shadow a GP or a hospital doctor if you can. But also volunteer in a social care setting. Speak to lots of different healthcare professionals, not just doctors. Have an open mind about whether being a doctor is the profession you want within healthcare, there are plenty of other interesting and valuable roles.
