I'm not particularly interested in the Clowns Paradise AU, but as I recall, Luffy is rubber in that one, so why would he have trouble shapeshifting? Do his powers simply not extend to that?
Yes and no.
Crocodile can shapeshift into pretty much anything be it humanoid or animal.
Luffy's body can be formed into different shapes but nothing too extravagant also... It's Luffy.
Anything that requires extensive detail would require focus and training sculpture to make that second nature. Unfortunately our rubber boy only sticks with simple or what feels right, anything else isn't important enough.
So he only has two "characters"
Lucy-bell, no one has any idea how this role came to be.
And a character manifested specifically to unintentionally re-traumatize his brothers, Zeroji.
Sanji thinks Luffy is just mimicking him which he finds adorable.

Making an au of an au again but Truthless is the were-clown... Mayhaps a little more malicious as he was sent to get rid of the king by an entity's contract for his own freedom.
Idea in question
An entity that collects anomalies to use for his deals, released Truthless on a servant's desire to see the king gone. However the wording of what he must do can be twisted...
The king suffers from continuous agonizing body pain which is only partially relieved through drinking excessive pain dampening potions.
The recluse feels a sort of twisted kinship towards the pitiful king.
