On the morning of Day 9, Leo woke wet and didn't panic.

"It's not that easy," Leo murmured.
"I know." She smiled, creases deepening around her eyes. "That's why I have a suggestion. There's another patient in the program — Jamie, nineteen. He's about ten days ahead of you, well into the full-acceptance phase. His mother has done the protocol with him very successfully. They've agreed to come over for a visit. A kind of… playdate, if you will."
Leo blinked. "A playdate?"
"Structured social time. You can play video games, build Legos, just hang out. No pressure. But seeing someone your own age navigate this same situation — without embarrassment, without hiding — can do more for your comfort than a dozen conversations with me or your mother." She leaned forward. "What do you think?"
Leo thought it sounded like the most mortifying experience imaginable. But underneath the dread was a sliver of curiosity. If someone else had really figured out how to not be embarrassed, maybe Leo could too.
"Okay," he said. "I'll try."
Jamie Kellerman arrived two days later on a warm Saturday morning.
He was lanky, with sandy hair that flopped over his forehead, a face that looked about sixteen, and an easy grin. He wore a baggy hoodie and basketball shorts, and when he walked, there was a faint rustle beneath the fabric. Leo heard it immediately. Diaper.

He set down his controller. His expression went slightly distant, eyes unfocusing. Then a soft, muffled hissing sound reached Leo's ears — the sound of liquid hitting absorbent padding. Jamie's posture was completely relaxed, shoulders slumped, one leg draped over the other. He was wetting his diaper. Right there. Without leaving.

"Goodnight Moon. I remember." She kissed the top of his head. "We'll read it after dinner."

He pulled on sweatpants, hoping the fabric would trap some of the smell. It wouldn’t. He knew it wouldn’t. But he couldn’t leave the room. Couldn’t have his mother see what he’d done. The thought was unbearable. So he sat on the edge of the bed, careful not to put weight on the mess, and waited.
The smell intensified. He stared at the wall and tried not to breathe.
Forty minutes later, his father came home.
Footsteps on the stairs. A pause on the landing. A sniff.
“Leo?” A beat. “You up here, buddy?”
Leo didn’t answer. His face was burning.
Dan tried the handle. Locked. “Leo, are you okay?”
“Yeah.” The word came out strangled.
Silence. Then retreating footsteps. Leo heard his father’s voice, muffled: “Marie? Can you come inside for a minute?”
Lighter footsteps. Quicker. A soft knock.
“Leo, sweetheart. Can you let me in?”
His mother’s voice. No judgment. No edge.
Leo stood. His legs felt unsteady. The diaper sagged, heavy and warm. He unlocked the door and opened it a crack.
The smell hit the hallway like a wave. Marie didn’t flinch. Her eyes went to his face, not his waist. “You did it,” she said softly. “Good. Let’s get you taken care of.”
She slipped inside. Dan had retreated downstairs, and Leo was grateful.
Marie guided him to the bed. “Lie down. We’ll get you cleaned up.”
The change was the most humiliating experience of Leo’s life, and also the strangest form of tenderness he’d ever known. Marie untaped the diaper — the tapes gave way with wet, heavy sounds. The mess was substantial, warm and mushy, filling the diaper from front to back. She used wipe after wipe. The cool cloths cleaned the mess from his skin, her touch steady and unbothered. She didn’t grimace. She didn’t pull away. She just worked, humming something soft.
“You don’t have to hide,” she said, folding the soiled diaper into a tight bundle. “I know it’s hard. But this is exactly what the doctor ordered. Your body is doing what it needs to do. And I’m never going to be upset about that.”
Leo stared at the ceiling. His eyes were wet.
“I was just…” His voice cracked. “It was so gross. I didn’t want anyone to see.”
“I understand.” She sprinkled powder into the fresh diaper. The scent cut through the lingering odor. “But you’re not gross. You’re healing. There’s a difference.”

That evening, Dan knocked on Leo’s door.

Dan crossed the room and set the mug on the nightstand. He stood there, shifting his weight, looking like he wanted to say something but didn’t have the words. His hand landed briefly on Leo’s shoulder.

“I don’t know why I can’t do it,” he says, and tears are starting to form in his eyes again.
“It’s okay! I didn’t mean to make it sound like you need to be able to get to the bathroom on time. It’s okay if you have another accident.”
Now we go in the bathroom and he sits on the counter while I wash his face. He squeezes his eyes shut and grimaces as I rub them with the washcloth.
“Let’s go out to the kitchen, okay? We can have a snack.”
He nods and grabs my hand as soon as he’s down from the counter.
We go into the kitchen and he sits down at the table, drawing his knees up to his chest. I open the cupboard and bring down a box of alphabet cookies and two sectioned plates. I get his sippy cup from the clean dish rack and open the fridge.
“Which juice do you want? Do you want the apple mango one? Or the cranberry one you got yesterday?”
“Cranberry,” he says quietly, and I take it out and set it next to the cookies and plates on the counter.
“I can pour it out myself,” he says quietly.
“No, not this time. You spilled yesterday, remember?”
He looks away, fidgeting with the tablecloth. I pour out cookies into both plates and also add some sugar snap peas from the fridge. I set his down in front of him, with his juice.
I sit down next to him and we start eating. He blushes every time I look at him while he’s drinking from his sippy cup.
“Does that feel good?”
“Does what feel good?” he asks, coughing a little at the end of the sentence.
“Drinking from your cup.”

“Are you feeling better? Or do you need me to come over there and sit you on my lap?”

“Okay.” I stand up and pull out his chair, helping him stand while I sit down. I pull him into my lap and wrap my arms around his waist.
We sit in silence for a few minutes while we both eat off his plate, until he suddenly squirms and tries to stand up, knocking his knee on the bottom of the table.

“Are you done?” I feel another trickle immediately, soaking down to my skin again.
He shakes his head, squeezing his legs together as the last few spurts come out.
“Now are you all done?”
He nods and I scoot the chair back, pulling him up under his armpits.
“I’m sorry…” he says again.
“You’re all good. Do you just need your nighttime diaper now? We could try one more pull-up, but it seems like it’s kind of hard for you today. That was two accidents in just about an hour.”
He wipes his eyes again and I pull him closer to me and brush his tears away with my sleeve. “It’s okay. We can do a diaper and PJs. You should take a shower first, okay?” I pull away from him. “Do you need help in the shower? Okay. I thought so.”
He slips his hand into mine again and we find ourselves back in the bathroom. “Let’s get these sticky pants off. I’ll take mine off, too. Oh, you got a little on my shirt. We have some laundry to do.”
He places the wet clothes in a pile on the tile floor and stands on the bath mat, rubbing one foot over the other and looking shy.
“Can you turn the water on and warm it up?”
He nods and turns around, bending over a little while he fiddles with the shower knobs.
“Okay, let’s clean you up first.” I reach for his monkey bath mit.
Once we’re finishing with the shower, I take him to our room and help him lie down on his changing mat, but not before pulling a PJ shirt over his head. “Do you want to go on the tablet while I change you?”
He nods, and I get it down from the shelf, along with a pacifier for him. He doesn’t protest at all when I sneak it into his mouth. I unlock the tablet with the password (that he doesn’t get to know) and pass it to him. He swiped through the apps a couple of times before finding the puzzle game he likes.
He’s a lot calmer as I rub cream along the insides of his thighs and his bottom. I can hear the very faint music and sound effects of his game. The sucking sound he’s making on the pacifier is a little louder.
“There. All done. Let’s get PJ pants on.”
He hands the tablet back to me and I put it back in its place on the shelf.
He makes sure I’m giving him matching PJ pants. He got really upset once when I changed him into a mismatched set when he was half-asleep.
“Are these ones good?”

Enter our protagonist. He's in line to be king in this society. There's one problem: he cannot for the life of him gain any weight. He's always been scrawny and skinny. Maybe his metabolism is just fast, maybe his thyroid is fucked; he has no idea, because they don't have that kind of modern medicine. But what he does know is that as is, he'll never get to ascend to the throne. Best case scenario, he gets asked to abdicate to one of his family members who are more than capable of taking the throne in his place. Worst case scenario, someone simply kills him. No one as willowy as him can rule. The entire kingdom would be a laughing stock.
Until one day, he gets backed up. He's never been especially prone to constipation. He's not familiar with the experience. But this time, maybe it's something he ate, or maybe his bowels just got in a bad mood, but he can't pass anything at all. As the days pass and he fails to get anything out, he begins to notice his waistline expanding. By the time his ailment has reached nearly a week, some of the people in the castle are looking at him with something almost bordering on respect. It appears to the outsider that for the first time in his life, he's actually managed to put some meat on his bones. And sure, a constipation bloat isn't exactly the ideal way to put on size—his arms still resemble twigs, and even his cheeks are hollow—it's the only option he has.
