Notice that Kara never told anyone at all that she was an alien before becoming Supergirl. Even before all the extra risks that come from being a superhero, Kara has never let something as "selfish" as her own desire to be open with people come before the safety of those she loves.

Usually I turn to Winn as a counterexample for Lena, but Kara didn't tell Winn for the sake of their friendship either. She told him because she had just lost her support from her sister and needed someone to open up to. Something huge just happened in her life and Alex was unwilling to be the person she could turn to to process it and figure out her steps forward. She needed Winn, and he was her best friend of three years so she did trust him to be that person she could turn too.

If Alex had been more supportive of Supergirl coming out, I highly doubt Kara would have told Winn unless something happened that made it absolutely necessary. (She probably would have pulled a season one Oliver and Felicity, with Winn just figuring it out in the end.)

Now Nia is a similar situation to Winn, Alex (and Winn) were out of the picture and Kara needed someone she could be open to. Nia was also already a half alien in a semi similar situation to herself, who could relate. And Kara also did it for Nia, who also needed someone to rely on then. Kara came out to help Nia, a friend, and a potential ally and mentee. Her relationship with Nia is very different from anyone else, so it's a different situation.

And Lucy is the obvious life or death situation, not much of a choice.

My first thought was Kelly or J’onn (since J’onn was the one who apparently made them switch up teams), but Kara looks too far left for it to be either of them. So it must be Nia! Do y’all remember the phone incident? Ofc u do:

This is the scene where Kara admits to Nia that she feels like she’s been a terrible friend to Lena.

So! Nia knows that things weren’t 100% peachy keen between Kara and Lena!

The gesture from Kara to Lena is to show “Hey Nia look at Lena and I being best friends again!” :)
