The way I see it, it’s an AU, because people don’t just get their eyes back like that. Seems Toby also pretty much confirmed this in his recent Twitter post (to paraphrase: different world, different characters, different lives; the UNDERTALE world is still the way you left it). Dunno what’s the cause for the difference, though. Maybe Chara never fell into the Underground.

What happens at the end is a more or less regular occurence, since the cage is described as rather battered and there is a stain at the floor. Sort of like a mental episode.
