It's the longest running gag in the show because Sam and Dean are compared to romantic couples throughout the entire series. Does that mean that they were fucking? Hell no, not in canon. But it was intended to point out how twisted up in each other they were. Sam and Dean are not supposed to come across as just a couple of normal brothers. Other characters in the show look at them and assume, that's the whole point.
