Of these, one-fifth are likely to have serious defects, requiring an intervention in the first year of life. Currently, advanced cardiac care is available to only a minority of such children.

The success rate of children's heart treatments varies depending on the type and complexity of the heart condition, as well as the type of treatment- catheter-based procedures or surgeries. Genesis Foundation, a children's heart foundation in India, provides critical support and access to life-saving child heart treatments. Till now, they have supported thousands of children battling with CHD with a success rate of 98% at their partner hospitals. In the financial year (FY) 2024-2025, they supported 410 children, with a 49-51% boy/girl ratio of beneficiaries. 43% of children were school going children, while 36 children from aspirational districts as identified by NITI Aayog, and 21 from the tribal belt.

Several cardiac care centers have come up in India over the last decade. The total number is just over 60; ten of these can be considered high-volume centers (more than 500 cardiac surgeries per year). However, most are in the private sector and are not geographically well located. The total number of births is much higher in North and Eastern parts of India as compared to the rest of the four regions (Southern, Western, Central, and North-East). Consequently, the total number of babies born with CHD is likely to be much more in regions with high birth rates. Pediatric cardiac care is a team effort involving cardiologists, surgeons, anesthesiologists, and intensive care specialists.

Recent data shows that 95% is the success rate achieved by the best hospitals in India, in complex pediatric heart surgeries. India’s infant mortality rate is currently 27 per 1,000 live births, compared to the global average of 19. Congenital Heart Disease is a major cause of preventable infant deaths. A substantial proportion of births in India occur at home, and the infant is likely to die before the critical, ductus-dependent CHD is diagnosed. Most children with CHD, including those who have undergone an intervention, require long-term care for a good outcome, which includes follow-ups as advised. Unfortunately, many children in India, especially those from middle or lower socioeconomic strata, are lost to follow-up due to financial hurdles or lack of awareness. Investment in healthcare is one of the lowest in India when compared with several other countries, including many LMIC (Low-Middle Income Countries). There is no national policy for CHD. Medical insurance is practically nonexistent in India, especially for birth defects. Most treatments are paid out of the pocket, leading to financial burden in families.

One heart-warming story from Genesis Foundation is of Pritika (name changed), who underwent a complex pediatric surgery at  one of their partner hospitals in Kolkata. She couldn’t get timely child heart treatment due to financial constraints as her father was an owner of a small grocery store. Nevertheless, with the help of their CSR partners, she continues to grow and heal.

How to Choose Between Surgery and Heart Hole Treatment Without Surgery?

The most important thing that can be communicated to parents or caregivers is that not all heart holes require surgery. Depending on the size, site, and presenting diagnostic symptoms, patients may be candidates for heart hole treatment without surgery. In many cases, small holes are subject to the following as part of a routine healthcare experience. Follow-up visits generally involve a review of echocardiograms and physical exams.
