In the comic, it shows followers of the cult with a white eye and one eye covered, too.

So I cooked up a theory. In order to join the cult, you need to follow Overseers path of enlightenment. So I think they join the cult and gain a white eye if they truly believe in the cult's mission. The reason they cover the other eye is because it isn't their "true" eye, given by the cult, to show they are devoted.

Medkit doesn't have a white eye because he ISN'T devoted. He lost his eye in the fight with Subspace, I know that, but couldn't he heal it? What if he did? What if, when he refused to accept the cult eye, they took an eye by force for his defiance? (Which made him unable to heal it again because of like, some magical hijinks and shenanigans or something.)

Dollmaker doesn't have a white eye, either. But she can't feel emotions properly. So, if she can't feel emotions properly, she can't feel TRULY devoted to the cult. The teal color of her visible eye could very well be a sign that is her "true" eye, and it only isn't pure white because she can't ever truly FEEL that devotion.

Now, Broker is the odd one out. He DOESN'T cover an eye. One is white and one is black. I think this is because he IS fully devoted. We've seen this to be true. Since he is so devoted, I think Father Overseer doesn't see any reason to force him to cover an eye anymore, because he CAN'T see the world without his "true" eye, and there isn't any worry that he will leave.
