After dying Dean meets Bobby, who tells him that Jack decided to change Heaven for the better, but he didn’t do it alone.

"Cas helped"

After learning that Cas is still alive, Dean looks for a way to get out of Heaven and find him, wherever he is, with the help of Charlie, Pamela and his family.

Or

Cas misteriously got out of the empty with Jack's help and now is nowhere to be found. After watching over him through a special Tv in his heavenly house, Dean strarts searching for Cas by traveling through portals, hoping to get him to come back to heaven.

Houses that change themselves, rooms that appear from thin air and TVs to watch over the living. With all these means, Dean learns how to reach Castiel and discovers why he's not in Heaven or in the empty. He's looking for something.
