I feel like a lot of people forget that Saki loves Tsukasa just as much as he loves her. He's just more doting, loud, and overbearing. He probably feels pressured (by himself more than anything) to be that way because of his role as an older sibling and the brother of a sister. This is before even mentioning Saki's illness in their early childhood. Given the chance, Saki would absolutely sacrifice anything if it was for Tsukasa's sake (she's also self sacrificing in general as we've seen in fading stars)
