The anime doesn't explicitly explain how or why Kinko loses her hand, but this is explained better in the manga/light novel

Anime Yuki brings up that if they try to lift the lever, then the handcuffs will tighten and they'll lose their hand. We see a brief scene where Kinko is pulling on the chain.

Light Novel Yuki sees Kinko's arm go all the way through to Momono's side to get the keys directly from Momono. Yuki realises that Kinko must have lifted the lever

Manga: In the manga we see things from Kinko's perspective. We see she's unable to reach the keys so she has to lift the lever
