My Heartbeat

Pedia ward rotation just started and I guess I can confirm na I'm a toxic clerk especially to the number of patients!  When I was on my second emergency pedia duty, I admitted 4 patients in a day.  And I disregarded the thought that I'm toxic because the my first ER duty, "petiks" lang. But now comes Pedia Ward rotation, my first post was in the Nursery/Delivery room.  Me and leslie were the first of the group to rotate in the nursery 24 hour duty.  Our first duty... 4 catches! Sabi pa ng intern and dra. wong na usually 1 or 2 catches per day lang.. We made it toxic!  And now comes the 2nd 24 hour duty... we had 6 catches!!!!! 3 of them were CS pa! Grabe! Pagdating namin ng morning we thought na benign pero came 10:30am.. may dumating na isa na malapit na manganak. After that may dumating na isa pa, na ANG TAGAL manganak... and siya ung na-emergency CS around midnight!  Then we had one catch naman na dumating 5cm dilated.. then 6-7cm dilated tapos biglang nagsisisigaw, and un! Good thing nailagay na siya sa table when the baby suddenly splurted out!!!! Grabe!!! Then after that na-cs na ung emergency CS then ung isang dumating ng 10pm para sa repeat CS! Ang question ko lang dun sa repeat CS eh WHY?!!? AT NIGHT! PWEDE NAMAN SA UMAGA! BAKIT KA PA SA GABI DUMATING! And then un... natira na alng ung isang patient na eventually i-CS din pala which we passed on sa next batch hehehe Grabe... we slept 3am na nun.. tapos walang from status kasi Fabella na ung next rotation namin.. so I'm awake for like 36 hours with just 1and1/2 hour sleep. I can't imagine pag nasa wards  na ako... baka ang toxic ko... huhhuhuhu and pano na pag nasa OB/GYNE rotation na ako!!!! GOOD LUCK NA LANG SA AKIN!!! HAHAHAHAHA

This post is long overdue. :P I took the remedials for Radiology back in third year because I found it really hard to digest everything that has been discussed.  That is why, I took Radiology as one of my electives for rotation as a clerk. Radiology rotation consisted of 6 stations, one station for each day.  They have Diagnostic Radiology, CTScan/MRI, Radiation Oncology, Ultrasound, Interventional Radiology, and Nuclear Medicine.  I learned most in Diagnostic radiology since this has the bulk of reading X-rays and what ALL general practioners should know.  All the I learned back in third year were reinforced.  Now I can say that I can be 90% sure when reading anXray plate.  Ultrasound was very interesting, and it kinda helped me review the anatomy, especially of the digestive and urinary organs.  Orientation to the different structures is important. Sad to say, but I learned a lot less on CTScan/MRI, it's complicated padin.. hehe We were quite lucky on Interventional Radiology because they had a TACE (transchemoembolization) procedure one day so we were able to observe it.  Kakapagod lang tumayo at mag-observe. Nuclear medicine, was very very interesting! It's one Radiology branch wherein you dont have to take residency in Radiology. so directly ka na mag-nuclear medicine resident. :P I'm tempted to get this as one of my specialty dream. And lastly, Radiation oncology.  It was very inspiring.  We saw this one kid patient for preparation.  He was very scared kasi. then there was this another kid who just is also for the treatment.  He served as a support group to the other kid.  Seeing that got me teary eyed.  I see radiation oncology as giving hope to cancer patients.  Parang yun na ung nagpapalakas ng loob nila, that there is still a chance of survival.. Oncology was one of the specialty that I was interested in before, but I changed my mind.  Maybe Radiation oncology will be a specialty that would pull me closer to cancer patients. Dr. Sarmiento, who is the one and only Radiation Oncology resident there, was very inspiring too with his words of wisdom :)

April 23, 2009, marked the end of our One week elective rotation in Anesthesiology.  And all I can say is that I enjoyed it and I learned a lot from it.  Although may pagka-benign (medical slang for light load) ung rotation, you'll really learn a lot from it.  The Consultants and Residents were great! Expert na expert in the field of Anesthesiology.  And the residents were all enthusiastic to teach! The consultants naman, kahit wala silang summer load, they were sill there to lecture us on some of the principles in Anesthesiology.  Idol ko sila! :D I wish I'll be like that when I become a resident and a consultant. Enthusiasm to teach. One advantage of Anesthesiology rotation is that you get to watch all kinds of surgeries! So far I saw... Mesh Hernioraphy, MRM, Total Thyroidectomy, Appendectomy, Tethered Cord surgery, ano pa ba... TURP, Laparoscopic Cholecystectomy, and a minor club foot surgery. I'm also happy with the rotation because we've seen most of the major anesthetic techniques like general inhalational anesthesia, general IV anesthesia, Spinal anesthesia, and Epidural Anesthesia.  Sana lang nakapagintubate kaming lahat, kasi isa lang sa amin ang nakapagintubate on a real patient. ;) I guess I have another option for specialization... Anesthesiology ;) Tomorrow, I'm off to RADIOLOGY! :D

Yup! Group 12 is on a 4-week Electives Rotation. For my first week, I chose Anesthesiology.  I'm with Leslie, Jen, Prerna, Robert, and Siegfreid. :D It was a very interesting one.  We mainly observed how the residents induce anesthesia and they made side lessons while doing so.  Sobrang daming information, but since it is on the practical side, medyo naretain lahat ng sinabi nila.  Saw some interesting cases today, like IV infusion anesthesia for the club-foot surgery of the Orthopedics surgeons, general anesthesia induction for a case of corneal abrasion/trauma, appendectomy, and laparoscopic cholecystectomy.  and a spinal anesthesia for a mesh herniorrhapy case.  First day, puro OR na :D It's kinda tiring because we basically had to go in and out of the OR and look for a consultant to accompany us, kaya naka-ilang palit din kami ng scrub suit namin! And can't believe that I'm getting the hang of it. Halos in less than 5 mins tapos na kaming magbihis! ahahah :D I hope this will be a fun 1-week rotation. :D

April 16, 2009, the dawn of the new chapter of my life.  I'm finally a 4th year Medical Clerk.  Can't believe I got this far, and I've been dreading for this day to come.  Honestly, I'm not really excited.  I'm scared, very very scared. I feel that I haven't learned that much, I feel that I'm not fully prepared to answer every consultant's questions.  But I'm already there.  And I guess I have to be stronger and face it.  This time, it will be different.  It will not be the books that would help us learn, but the patients.   It will be more challenging, and I guess there will be competition among the medical clerks.  But I'm hoping that this would not stop us to work together as a team in the hospital.  This is it! There is really no turning back! Just a little more 365 days, I would be taking the Hippocratic Oath, officially.  Can't wait for April 15, 2010, where Clerkship ends.. and we graduate after :D I'm looking forward to that day.
