As a result of some incident, Boyo has gained the ability to turn back and forth into a Twisted (Lethal) just like Dandy and Dyle can do, except Boyo has full control over his Twisted form. As a Twisted, Boyo just moves around the entire floor, not bothering any of the Toons and just keeps to himself. There are times when he'll just follow around the toon with the lowest health, but if more than one toon has low health, then he'll choose one of them randomly based on a stat difference like speed, stamina, etc. on who has the lowest between them. Boyo doesn't harm any of his targets, but instead he stares at them, especially when they're at an ichor pump. Eventually, he'll leave before doing one of two things: He'll either return with a healing item and leave it next to the toon or distract any nearby Twisteds from coming near them.
There are times when he'll go and bother the Twisteds by either distracting them or physically stop them from harming a toon they're chasing. Boyo is strong enough to hold back both small and tall Twisteds as they're easy for him to handle, but big Twisteds like Dandy, Dyle, Pebble, Bobette, and Coal are ones he'll struggle against. If strength doesn't work against the big ones, then he'll distract them by either tricking them into going to a different part of the floor away from the toons or by getting them to play with him.
Twisted Boyo's emotional state depends on how Boyo himself currently feels. If he's sad, then he'll go off to a random part of the floor and wants to be alone, not bothering anyone and will be fine as long as no one messes with him. It's a whole different story if he's mad as being in emotional state of rage will cause him to go on a rampage and target the toon with the lowest hearts, which makes him dangerous to any toons who needs to be healed. Whether he's sad or mad, Boyo's Twisted form can cause some serious trouble.
Also, Boyo's Passive Ability, 'Sacrificial Heart', was replaced with an ability called 'Twisted Turn'. When this ability activates, he turns him into his Twisted form, which lasts for only one floor. As a result of him losing Sacrificial Heart, he can no longer sacrifice one of his hearts to protect his allies anymore. In this form, he's faster and stronger than he was before.
Twisted Boyo's ability depends on his current negative mood:
Tears of Ichor (Sad): Boyo lets out a crying roar before ichor rains down from the ceiling and throughout the entire floor. It inflicts a Slow II effect onto the toons. The ability ends either when Boyo feels better or when the toon who Boyo trusts calm him down.
Threads of Hate (Angry): Boyo lets out an angry roar before streaks of ichor appear in random places of the floor and walls, and out of them are threads made out of ichor that are set in different directions, length, and angles depending on the location of the streaks themselves. The effects of the strings that a toon comes into contact with are a random 50/50 chance between two as the strings will either wrap around the toon, restraining them, making the toon waste some of their stamina escaping, or it sets off a loud sound that not only disorientates the toon but also attracts the attention of nearby Twisteds at the same time. The ability lasts until the next roar, which will reset the placement of the streaks and the angle directions of the strings.
