My approach is to look at Truthless with the same sort of aura as someone who has survived a great tragedy, but lost all that was important to them in the process. Something like a dancer who was in a terrible accident, but lost their ability to move, or an artist who can no longer use their arm.

He devoted his life to searching for the truth that would let him help those around him, the truth Pure Vanilla discovered after braving the Sugarfree Roadand discovering his Souljam, but unlike his counterpart, Truthless found NOTHING. And if he was similarly leading others who chose to put their faith in him at that point, it's not hard to imagine that they suffered greatly for supporting and following him and likely either lost their lives or abanoned him in anger when he failed to deliver what they hoped for, just like what we saw in the false visions Shadow Milk gave Pure Vanilla at the spire.

And yet rather than lashing out at others or putting the blame elsewhere for what happened, Truthless takes all the burden upon himself with grief and self loathing, choosing to stand guard to stop others from experiencing the same pain that he suffered.
