some neuroscience thoughts
I think that the chemical imbalance theory of depression is wrong. I donât think thereâs one or two neurotransmitters that account for depression. Itâs why antidepressant really suck at working. Depression can be caused by numerous things ranging from genetic predisposition to stressful life circumstances, and all of these are probably encoded in specific circuits in the brain rather then a disruption in the entire chemistry of the brain. What I like about this concept, is that the brain is very plastic and although it can be difficult these circuits can be rewired. Everyone has the potential for recovery, it just takes the right environment and resources to get there. Mind over matter.
Thank you.Â
Six, I love you, but someone is going to use this post to tell someone to âget over their depressionâ and to just âthink happy/think the sadness awayâ as if depression is some self-inflicted booboo.
I hope no one does that because telling someone to âget overâ something and to just think about rainbows and unicorns is an awful idea. You canât rewire your brain in a day
It takes time but through the right resources (support group or encouraging friends and family, meditation, exercise, better diet, therapy etc.) recovery will come through.Â
Also hereâs a good article on why antidepressants arenât the best but why theyâre so popular:Â https://www.newscientist.com/article/2093239-if-antidepressants-dont-work-well-why-are-they-so-popular/
PS: Itâs not that antidepressants are completely usless, they work miraculously for some people- but thatâs the problem itâs only some people. For others it can have awful side effects and not help at all. It tells us that through these pills weâre not really getting to the heart of the issue. Thereâs more to the intial problem then changing the concentration of one or two chemicals.
I actually learned about this in my Psych 101 college course. Thereâs different approaches to mental health, some focus on the chemicals and medical and others on behavior or life experiences. What modern experts have found is that a combination of therapy (talk, CBT, etc), lifestyle changes, plus medications is most effective. Some people may never be able to fully be off medications because their brain chemistry is severely imbalanced and thatâs fine. Others need it to help cope while therapy and lifestyle changes start to take hold. And then some people can just use therapy and no medications. Your brain chemicals can also fluctuate so dopamine may be your issue for a while then over time serotonin is the issue (as an oversimplified example). This is why itâs important to check in with a doctor regularly. Much like having to fine tune treatment for things such as diabetes or asthma, mental healthcare is highly specific and changes over time.
Hey! We medical folks have noticed this and have been scrambling to get something called Comparative Effectiveness Research accomplished. Instead of just being a study with x amount of evidence that drug A works, it looks at multiple therapy courses and their relative efficacy to one another, and even examines the individual patient factors (race, age, sex, other illnesses, etc etc) that contribute to hopefully guide you and your doctor to the therapy that I'll work best for you! It'd a growing field and we need more people to volunteer their info research as well as more people to become researchers. We also need more hospitals to adopt more modern computer systems to facilitate data creation, capture, and analysis. You can contribute to that last one by paying attention to legislative matters pertaining to healthcare (look up HITECH for an example, as well as meaningful use). Medicine has only really pushed for scientific basis in the last hundred years. The push for scientific (rather than anecdotal) evidence in medicine was really initiated and made standard in the 1980s. We have a shit load of work to do but we need help. Love, A nurse

Take care of yourself today.
When youâre depressed (or anxious, or triggered), staying in all weekend, not answering the phone, binge-watching TV, and not getting dressed sounds great. It might even sound like âself-care.â And aspects of it can be self-care. But self-care is not just about soothing yourself in the moment, itâs about setting up the supports and structures that let you be okay enough in your day-to-day life. So while depression says âletâs watch Buffy instead of doing the laundryâ the reality is that tomorrow youâre going to wake up to clothes everywhere, nothing clean, and one more thing you havenât doneâwhich will add to the guilt and shame that seem to come hand-in-hand with depression. On the other hand, depression-challenging behaviours are hard and not fun in the moment, but set you up to a) have small victories (SO important when dealing with mental health issues), b) have some structure and routine in your life, and c) set up the support and structure to let you deal with the root of your issues or cope with issues that arenât going away anytime soon.
