Im sorry if you wouldn't like it, but i have a headcanon for Painted Smile. I just read it and i fall in love with this masterpiece.
My headcanon is that when they were 15-16 y.o. they tried to kiss. Idk the reason, maybe they read smth where prince and princess kissed, but it must be very funny. This awkward kiss, just one because they were too embarrassed. They never spoke about it again.
Im really sorry. Pls don't kill me for this. I just can't stop thinking about Alastor taking her very first kiss. It's sweet and calm. I love it. You can just ignore it!!!

It's a very cute headcanon, I agree with you. Alastor doesn't need affection from other but his Mother. But he did begin to crave reader's attention just like reader wanted to stay with Alastor!
I think Reader would be the one to bring the topic, asking if he ever kissed someone. Alastor would just say no, as he didn't really care about it but maybe Reader was feeling insecure about her inexperiment about love in general so Alastor asked her is she wanted to be kissed. With a shy nod from her, Alastor would kiss her and as you said it was very embarassing for the both of them. Reader would hide under the cover as Alastor would just be confused but content at her reaction.
I think it would have happened like this...Maybe.
