This form is for existing participants in the long-layaway preorder for twigling dolls, that has been running since January 2015 (please go here: www.twigling.com/2015-payment-plans/ to sign up). This sheet aims to collect more detailed information from participants in an organised manner that will give myself and the factory a clear overview of what is to be produced. It also helps to determine the final cost of each order so that your total can be adjusted accordingly. This format has some limitations, but I hope it will help us to collate the information, even if I still have to make some calculations manually. I thank you for your patience and for the learning opportunity it has been to run this order, hoping quite insistently that the process will be cleaner and more streamlined in the future. If you have ordered more than one doll, please fill the form in once for each doll. If you want to order just a head, or heads or other parts, please wait for a separate order form to be provided, but feel free to contact me on facebook.com/vrouwkijn or [email protected] to let me know you wish to order separate parts. As always, if you have any questions, please direct them either to my email or message me via the Facebook page, both linked in the above paragraph. If contacting me via either channel, please identify yourself with full name and email address so I can localise information relevant to your order. NB! Do not use the "Back" button in your browser to go to the previous page on the form, instead use the buttons built into the form. Otherwise your entries will not be saved and you will have to start over.

If you would like to participate before the order closes at the end of August, please fill it out and I will contact you to organise payment instalments etc. You do not need to pay the deposit beforehand if you want to set up your payments in a different format.
