I have a headcanon that Sam self-harmed when he was hallucinating Lucifer, because he started using the pain from the cut in his hand to ground him in reality, which I thought might escalate to self-harm in the show but didn't, so it's my headcanon. idk if this is relevant but yeah/
I have the same headcanon.
There's a big difference between how teenagers deal with pain and how adults deal with pain. When Dean was in hell, he was 30. Dean in that story was a teenager without coping skills or life experience. And we don't know for sure that Dean didn't self-injure after hell either. It's all just speculation and headcanon.

I'm trans*, and the dysphoria can get so bad that you self-harm. It's not so out of the box. At least, not to me.
