So I have two fun ideas for what Kayne could look like.
•He definitely has skin, but it doesn't look used enough. There are creases from movement, but the lines don't have the depth they should. There aren't any scars. He has some of the imperfections of skin, but at the age he looks like, there should be more. It isn't completely uncanny, maybe he has something a little wrong with his nerves. Kayne doesn't move enough to create the right forms of wear. He teleports too much and he doesn't touch things. No one would could point it out as wrong, but it isn't right.
•He's covered in scars. Most of them look new and none of them look kind. He's fundamentally asymmetrical. He's kept the blood on his hands, why not other signs of his fights? He's proud of killing the other Nyarlathoteps. Look at his achievements over the whole of him. He's ragged and bloody with victory. And in a way, he and Arthur match. He's a different kind of god. So much more real than the other ones. He's awake unlike them. He can bleed and he'll force the same thing onto them.
