A modern AU where everything is the same but is the present time and HRE instead of falling he have a car accident and dies, then Germany another kid who were born with a heart condition receives the heart transplant from HRE. Then 13 years later he travel as a transfer student to Italy where he a bit shy but decided have a degree in world history, then he met his roomate Feliciano, an odd looking student who don't look that fit on the class at all but still is too friendly, Italy follows Germany everywhere in the campus and ask for his friendship, Germany who dont have any friends in college accept his friendship, then Japan, the roomate next door met them on classes and accept to help them on class. Things get weird at night when Feliciano hop into Germany's bed and hug him from behind while he make strange noices ("Ve") Germany at first didn't like the idea but it never bothers him, maybe he thought that Italy felt lonely and he quickly get used to Italy's strange way of friendship, then something in his heart start pounding, every night that Italy sleep with him or take a shower with him (Italy say that is to save time) he feels something familiar, as if he met him before from a past life, maybe it was for the heart transplant thet he received when he was a kid? He will never know.
