1 - we know that demons, especially azazel, love corrupting the pure, and the actor who plays azazel literally says that sam's heart was the purest, and therefore why he was so fixated on corrupting him.
2 - azazel, in feeding sam his blood, and through mary's deal with him, kind of makes sam his father, and it's a violating thing that he did against mary and her family.
3 - in the original meaning of the song, he refers to the older brother and you refers to the narrator's older abuser. maybe dean hates the way that azazel looks at sam, maybe john hates the way azazel looks at sam. (but this also makes me think of john & sam and how sam's "father" is azazel and sam is not completely john's kid, and john knows this every time he looks at sam.)
4 - sam's been doomed since before he was even born.
