Because the curse (blackness) causes tissue necrosis, the curse has to be vented in black feathers on the head. The longer the feathers and the more of them there are, the greater the damage of the curse. You can't just pluck and throw away the feathers - not only is it useless, it's also dangerous for those around you, since the curse can spread. And it's useless because a sick mind generates its own curse, and this same curse worsens the mental state. A vicious circle. The curse can be safely and temporarily removed with fire, but it must be magical fire from Sirius herself. Fire arises from Emotions with which she has a bad relationship, she does not experience them. And she is not immune to fire. Sometimes, of course, she manages to cleanse herself, but this is very rare.
