At one point when Leonard was tattooing himself I started wondering “when did he learn to do that? How does he remember how to do that?” And then I remembered how Leonard had mentioned that Sammy Jankis could do complex tasks as long as he had learned them before his injury... and while tattooing and administering an insulin shot aren’t the same, Leonard certainly must have learned how to sterilize a needle and where to prick himself and how if his wife was in fact a diabetic... 

I still maintain that Teddy was lying when he claimed “Sammy didn’t have a wife,” but I don’t think he was actively lying- I think he was misremembering or didn’t know the entire story. It just doesn’t make any sense for Leonard to have created such an elaborate story if he never met Sammy’s wife, or if Sammy didn’t even have one. Plus I feel like the guilt he carries due to the Sammy Jankis case just works really well with the narrative.
