This is something that came up tonight at EDH night. Player A had a Spellskite and some other valuable enchantment. Player B cast Sylvan Reclamation targeting the Spellskite and the other enchantment. Player A activated the Spellskite ability to save their enchantment. What happens when the ability resolves? Can Spellskite make itself a double-target of the Reclamation? We went back and forth on this for awhile, bringing up cards like Kolaghan's Command and Newlamog's trigger.
When Spellskite’s ability resolves, it won’t be able to change any targets of Sylvan Reclamation to itself because it’s already a target of that spell. Sylvan Reclamation can only target the same object once because it only uses the word target once.
Kolaghan’s command is a different case since the different modes each use the word target and so Spellskite can be targeted by both the damage and artifact destruction modes of that spell. Ulamog, the Ceaseless Hunger’s trigger works the same as Sylvan Reclamation and can only target Spellskite once.

I have a single Falkenrath Gorger on my side and a heir of falkenrath in my hand. My opponent cast kolaghan's command dealing 2 damage to Gorger and making me discard. Can I play the Heir of falkenrath with madness before the Gorger dies?
Not quite, but you still get to cast it for its madness cost granted by Falkenrath Gorger.
Falkenrath Gorger hasn’t taken any damage when you discard Heir of Falkenrath, so it will be discarded to exile due to its madness ability. And that ability will trigger. Falkenrath Gorger will be dead by the time you put the Madness trigger on the stack, but at that point it doesn’t matter anymore. You’ll be able to pay the madness cost when the triggered ability resolves anyway.
